UC DAVIS: BOOKSTORE -- MEMORIAL UNION September 30, 2003 DEANS, DIRECTORS, DEPARTMENT CHAIRS, AND CAMPUS ADMINISTRATIVE OFFICERS Re: Rental Form for Academic Hoods, Fall Commencement 2003 All faculty and administrators with a degree OTHER than a Ph.D. from the University of California who plan to take part in the Fall Commencement ceremony and want to wear an academic hood must make reservations through their department office for academic hood rentals by Oct. 31, 2003. Please fill out and fax in the attached generic form or send a request for your department’s tailored form*. Rental fee is $20. A separate rush-handling charge of $10 per hood will be added to all hood orders received after Oct. 31. Hoods will be delivered to the campus address listed on the form. Faculty members should return the hood at the collection station at the ceremony or to the person in their department who requested it for return via campus mail. IMPORTANT NOTE: DO NOT USE THIS FORM FOR: Faculty Hoods for a PhD from any UC campus or for Faculty Gowns. These items are supplied free of charge by the General Rental Service through Central Stores and separate ordering instructions will be distributed by them. *If you have placed past faculty hood orders, a department-specific form with your faculty info already completed is available for your convenience and can be faxed to you.
